This work is based on theoretical and clinical concepts that allow for a comprehensive understanding of the approach to this condition, strengthening the knowledge necessary for its proper management. The frequency with which these cases occur motivated the development of this review, in order to delve deeper into the diagnostic, orthodontic, and surgical aspects involved in the treatment of these patients. In this way, basic and specialized knowledge related to maxillary correction procedures used in patients with skeletal Class II malocclusion are integrated. The comprehensive treatment of these patients generally begins with an orthodontic phase aimed at achieving a proper dental relationship. Subsequently, the corresponding surgical intervention is performed according to the characteristics of the anomaly present. Finally, the patient continues with post-surgical orthodontic treatment, with the objective of optimizing the results obtained and ensuring long-term functional and aesthetic stability. Some complementary therapeutic alternatives will also be mentioned. However, the emphasis of this monograph is on the presentation of the clinical case and the description of the orthodontic and surgical management implemented.Resumen La presente monografía tiene como propósito brindar un enfoque clínico sobre el manejo de pacientes con maloclusión Clase II esquelética que requieren tratamiento de cirugía maxilofacial. Este trabajo se fundamenta en conceptos teóricos y clínicos que permiten comprender de manera integral el abordaje de esta condición, fortaleciendo los conocimientos necesarios para su adecuado manejo. La frecuencia con la que se presentan estos casos motivó el desarrollo de esta revisión, con el fin de profundizar en los aspectos diagnósticos, ortodónticos y quirúrgicos involucrados en el tratamiento de estos pacientes. De esta manera, se integran los conocimientos básicos y especializados relacionados con los procedimientos de corrección maxilar empleados en pacientes con Clase II esquelética. El tratamiento integral de estos pacientes generalmente inicia con una fase ortodóntica orientada a lograr una adecuada relación dental. Posteriormente, se realiza la intervención quirúrgica correspondiente según las características de la anomalía presente. Finalmente, el paciente continúa con el tratamiento ortodóntico posquirúrgico, con el objetivo de optimizar los resultados obtenidos y garantizar la estabilidad funcional y estética a largo plazo.
