Although previous studies have evaluated this relationship, longitudinal evidence remains limited. Objective: Evaluate the position of the lower lip and the axial inclination of the upper central incisors during growth in patients with Class I, Class II division 1 and Class II division 2. Methods: An observational, cross-sectional, and analytical study was conducted using lateral cephalometric radiographs from the AAOF Craniofacial Growth Legacy Collection database. Patients with Class I and Class II divisions 1 and 2 malocclusion were included according to established criteria. Collum angle and lower lip position relative to the maxillary central incisor were evaluated using WebCeph and ImageJ software. Intra-examiner calibration showed agreement (ICC and Kappa = 0.99). Statistical analyses were performed to determine the relationship between the variables. Results: Sixty patients were evaluated, distributed into three skeletal groups. Each subject provided four follow-up lateral radiographs, for a total of 240 observations. The position of the lower lip was predominantly incisal (57%), decreasing with age. The collum angle showed significant differences between skeletal classes (ANOVA, p<0.001), being greater in Class II-2 and negative in Class II-1, increasing progressively with age. Conclusions: There is a significant relationship between the lower lip position and the Collum angle during craniofacial growth.Introducción: La posición del labio inferior y el ángulo de Collum pueden influir en la inclinación de los incisivos superiores y en la estabilidad ortodóntica, especialmente en pacientes con maloclusión Clase II división 2. Aunque estudios previos han evaluado esta relación, la evidencia longitudinal siendo limitada. Objetivo: Evaluar la posición del labio inferior y la inclinación axial de los incisivos centrales superiores durante el crecimiento en pacientes con Clase I, Clase II división 1 y Clase II división 2. Métodos: Se realizó un estudio observacional, transversal y analítico utilizando radiografías cefalométricas laterales de la base de datos AAOF Craniofacial Growth Legacy Collection. Se incluyeron pacientes con maloclusión Clase I y Clase II divisiones 1 y 2 según criterios establecidos. El ángulo de Collum y la posición del labio inferior respecto al incisivo central superior fueron evaluados mediante los softwares WebCeph e ImageJ. La calibración intraexaminador mostró concordancia (ICC y Kappa = 0,99). Se realizaron análisis estadísticos para determinar la relación entre las variables. Resultados: Se evaluaron 60 pacientes distribuidos en tres grupos esqueléticos. Cada sujeto aportó cuatro radiografías laterales de seguimiento, para un total de 240 observaciones. La posición del labio inferior fue predominantemente incisal (57%), disminuyendo con la edad. El ángulo de Collum presentó diferencias significativas entre clases esqueléticas (ANOVA, p<0,001), siendo mayor en Clase II-2 y negativo en Clase II 1, aumentando progresivamente con la edad.
