Based on a meta-analysis, Apronax was established as the allopathic medicine to be used. To determine the homeopathic remedy, a pilot study was conducted with eight patients who attended the Chapinero Clinic of the COC with a recommendation for extractions of impacted third molars. Arnica was identified as the homeopathic remedy of first choice. Once this was established, the definitive study was conducted, involving patients who attended the COC's Outpatient Surgery Clinic and the Palermo Clinic. The surgical procedures were managed by two surgeons. Three patient groups were established based on the type of treatment they would receive. Group 1 received homeopathic remedies, group 2 received homeopathic and allopathic remedies, and group 3 received allopathic and placebo remedies. Once the data were collected, statistical analysis was performed to measure the effectiveness of the two types of medications in pain management. This study presents a new option for the therapeutic management of postoperative pain in impacted third molar surgery.Ante la necesidad de encontrar nuevas alternativas para el manejo del dolor postoperatorio en cirugías de terceros molares incluidos; se realizó el estudio de tipo cohortes con un diseño Doble Siego; partiendo de la lectura de un meta análisis y estableciendo el Apronax como medicamento alopático a utilizar. Para determinar el medicamento homeopático, se elaboró una prueba piloto con 8 pacientes que asistieron a la Clínica Adscrita de Chapinero del C.O.C. con indicación de exodoncias de terceros molares incluidos; identificando el Árnica como medicamento homeopático de primera elección. Una vez establecido, se procedió con la realización del estudio definitivo para el cual se contó con pacientes que llegaron a la Clinica de Cirugia Ambulatoria del C.O.C. y la Adscrita de Palermo. Los procedimientos quirúrgicos fueron manejados por dos cirujanos. Se establecieron 3 grupos de pacientes, de acuerdo al tipo de tratamiento que iban a recibir. Al grupo 1 se le suministro Homeopático, al 2 Homeopático y alopático, y al 3 alopático y placebo. Una vez se recolectaron los datos, se realizó el análisis estadístico para medir la efectividad de los dos tipos de medicamentos en el manejo del dolor.
