The aim of this systematic review was to evaluate the scientific evidence about the effectiveness of early treatment (<10 years) on mandibular growth with functional applications for Class II malocclusion. METHODS. We conducted a systematic search of the following databases: MEDLINE, CENTRAL, EMBASE and CINAHL, as well as congresses memories and contacted experts. MeSH terms used for the search were "Orthodontic Appliances, Functional" [Mesh]) AND "Malocclusion, Angle Class II" [Mesh]. Two independent reviewers performed the search and selected relevant articles independently. Sagittal mandibular position (SNB and /or ANB), mandibular total length (Co-Gn, Co-Pg and / or Ar-Gn), mandibular body length (Co-Gn) and, Go-Gn y Ar-Gn were taken into account for analysis. RESULTS. 328 potential articles were selected. According to inclusion and exclusion criteria, 8 papers were selected for analysis. Six reported changes in mandibular growth, only two studies reported significant changes in the distance Go-Pg, which showed a statistically significant increase. Five studies reported changes in SNB, of whom three had significant increase in this measure..CONCLUSIONS. No studies of high quality were found. Differences in outcomes measured in each of the studies included were detected. Most studies show an increase in mandibular length with early functional treatment, however it requires further investigationOBJETIVO: Evaluar la evidencia científica acerca de la efectividad del tratamiento temprano (<10 años) sobre el crecimiento mandibular con aplicaciones funcionales para mal-oclusión clase II. MÉTODOS. Se realizó una búsqueda sistemática en las siguientes bases de datos: MEDLINE, CENTRAL, EMBASE Y CINAHL, además de memorias de congresos y contacto a expertos. Dos evaluadores independientes realizaron la búsqueda y seleccionaron los artículos relevantes de forma independiente. Se tomaron en cuenta posición sagital mandibular (SNB y/o ANB), longitud total mandibular (Co-Gn, Co-Pg y/o Ar-Gn), longitud del cuerpo mandibular (Co-Gn) y las medidas Go-Gn y Ar-Gn. .RESULTADOS. 328 artículos potenciales. Después de realizar la selección de acuerdo a los criterios de inclusión y exclusión, 8 artículos fueron seleccionados para el análisis. 6 reportaron cambios en crecimiento mandibular, sólo 2 estudios reportan cambios significativos en la distancia Go Pg, de los cuales uno mostró aumento estadísticamente significativo. 5 estudios reportan cambios en SNB, de los cuales 3 presentaron aumento significativo en esta medida. CONCLUSIONES. No se encontraron estudios de alta calidad y se encontraron diferencias en cuanto a los desenlaces medidos en cada uno de los estudios incluidos.
