In 1844, Farrar described a radical and heroic treatment for alveolar abscesses involving root and tooth amputation, and in 1897, Rhein recommended root amputation as a radical cure for chronic alveolar abscesses. In 1886, Black also discussed the total amputation of individual roots in molars severely affected by periodontal disease and root canal treatment for the remaining healthy roots. In 1919, Garvin confirmed the possibility of apical filling with radiographs. Today, endodontic surgery yields good results and is often necessary. Part of the success of this surgery is due to its simplification. We must dispel the concept that endodontic surgery is a treatment, since it has the same objective as conservative surgery; It is not radical since the objective of these surgeries is to save the tooth, not to extract it.La cirugía endodóntica se inició muchos años atrás, cuando un médico y odontólogo griego empezó a investigar al absceso apical y cómo incidir por medio de cirugía. En 1844 Farrar describió un tratamiento radical y heroico del absceso alveolar por medio de la amputación de raíces y dientes, y Rhein 1897 aconsejo la amputación de raíces como curación radical del absceso alveolar crónico. En 1886 Black también habla sobre la amputación total de las raíces individuales de molares muy afectados por enfermedad periodontal y tratamiento de conducto en las restantes raíces sanas. En 1919 Garvin confirma con radiografías la posibilidad de obturación por vía apical. En la actualidad la cirugía endodóntica da buenos resultados y suele ser necesaria. Parte del éxito de esta cirugía se debe a su simplificación.
