La cirugía del tercer molar es el procedimiento ambulatorio más común realizado por cirujanos orales y maxilofaciales. Los enfoques quirúrgicos para la extracción de terceros molares se han publicado desde el siglo XX. Este artículo revisa la historia y el desarrollo de las técnicas de extracción a través de una revisión de la literatura. La literatura fue seleccionada a través de una búsqueda en una base de datos electrónica. Las palabras clave para la búsqueda en Medline fueron "tercer molar mandibular/maxilar", "tercer molar mandibular/maxilar impactado", "diseño de colgajo del tercer molar mandibular/maxilar" e "incisión del tercer molar mandibular/maxilar". La búsqueda se restringió a artículos en inglés. Además, se realizó una búsqueda manual en las principales revistas y libros de cirugía oral. El objetivo de este artículo es examinar la evolución de la cirugía del tercer molar, reconocer técnicas pioneras y comparar estas técnicas con los enfoques actuales. Se discuten los enfoques comunes empleados hoy en día y se proporcionan filosofías de tratamiento con ideas para futuras terapias.
Abstract:
Third molar surgery is the most common ambulatory procedure done by oral and maxillofacial surgeons. Surgical approaches for the removal of third molars have been published since the 20th century. This article reviews the history and development of extraction techniques through a literature review. The literature was selected through a search of an electronic database. Key words for the Medline search were “mandibular/maxillary third molar,” “impacted mandibular/maxillary third molar,” “mandibular/maxillary third molar flap design,” and “mandibular/maxillary third molar incision.” The search was restricted to English-language articles. Additionally, a manual search in the major oral surgery journals and books was performed. The aim of this article is to examine the evolution of third molar surgery, recognize pioneering techniques, and compare these techniques to current approaches. Common approaches employed today are discussed and treatment philosophies with thoughts for future therapies are provided.
